Appendix 3. Patient Data Collection Card Templates
Attached are templates for the traditional "tan-yellow-blue" data collection
cards. They should be pretty self-explanatory. The tan card is to record
basic patient data, history, and physical exam findings upon admission.
I have added an ROS checklist since I can never recall it on the fly.
The yellow card is to record labs and studies; the old card split the
labs up into two separate sections, which made it harder to tell at a
glance which labs were done on the same day. The blue card is for pre-rounding
and daily care planning; I've doubled the size to allow easier recording
of A&P and scut notes. Also, I've changed the cards to 4x6" dimensions,
so they will be about the same size as standard large-size index cards.
Consider the cards as "training wheels" to get you used to data collection.
Once you're in the habit, you'll have a mental image of the templates
and may no longer need the actual cards.
These templates can be submitted to Kinko's or similar for Xeroxing onto
card stock. You have to create the "mock-ups" yourself, or Kinko's will
charge you a boatload. A mock-up should have the fronts and backs of the
templates lined up exactly as they should align on the card stock before
it gets cut into separate cards. To save paper, two copies of the template
can be placed on a page, going horizontally rather than vertically as
was done here.
By the way: You can make them any color(s) that work for you.
