MG Status - Medical group membership status
  Active - A: Non-hospital based, full-time, salaried, clinical faculty who should appear in directories
  Active - B: Volunteers or part time clinical faculty
  Inactive: Providers membership is not current, recredentialing with the Medical Group is necessary
  Applicant: Application in progress, Medical Group memebership is pending


B&T Status - Brown and Toland Medical Group status
  Active - A Enrolled with Brown and Toland, meets Medical Group criteria as an "Active - A" provider and appears on the BTMG roster
  Active - B Enrolled with Brown and Toland as approved UCSF provider, but does not appear on BTMG roster of providers
  Active - HB Enrolled with Brown and Toland Medical Group as a provider who's services are hospital based. These providers will not be listed on the Brown and Toland provider roster
  Inactive Providers membership is not current, recredentialing with the Brown and Toland is necessary
  App In Process Application in progress, Brown and Toland memebership is pending


Status - Status of medical staff priviledges
  Active Provider has current Medical Staff Privileges
  Active (P) Provider has current Medical Staff Privileges with Proctoring requirements
  Applicant Provider is currently applying for Medical Staff Privileges


Category
  Attending: Physicians, dentists, podiatrists or clinical psychologists who are involved in patient care and/or in the supervision of students or house officers in their involvement with patient care or contact must be members of the Attending Staff. Members of the Attending Staff who have not been involved in patient care at the Medical Center and who have not been involved in the clinical supervision of students or house officers at the Medical Center for a period of two (2) years shall automatically be transferred to Inactive Status.
  Courtesy: Physicians, dentists, podiatrists or clinical psychologists who admit five (5) or fewer patients per year or devote less than 150 hours per year to patient care activities at UCSF Medical Center may apply for appointment to Courtesy Staff. Members of the Courtesy Staff who have not been involved in patient care at the Medical Center and/or who have not been involved in the clinical supervision of students or house officers for a period of two (2) years shall automatically be transferred to Inactive Status.
  Affiliate: Affiliate Staff shall consist of those physicians who were formerly on Medical Staff at UCSF/Mount Zion Medical Center prior to December 31, 1999, who have not completed full training in their specialty and/or do not meet board certification or eligibility for board examination, but who, nonetheless, appear likely to provide a distinct benefit to the Medical Center, the Medical Staff, and patients. The Affiliate Staff category shall expire automatically upon the cessation of medical staff privileges of the last Affiliate Staff Member who qualifies under this section.
  Allied Health: (AHP) means and individual, other than a licensed physician, dentist, clincal psychologist or podiatrist, who provides direct paitent care services in the Medical Center under a defined degree of supervision by a Medical Staff member who has been granted clinical privileges. AHP's exercise judgement within areas of dicumented profiessional competence and consistent with the applicable State Practice Act. AHP's are designated by the Governing Body to be credentialed and privileged through the Medical Staff Organization and are granted clinical privileges as defined in these Bylaws and related policies and procedures. The Governing Body periodically determines the categories of the individuals eligible for clinical privileges as an AHP. These categories are listed in the AHP Credentialing Policy and Procedure. Allied Health Profesionals are not eligible for Medical Staff Membership.
  Visiting Privileges: In circumstances in which patients or an academic program require the services of a physician, dentist, podiatrist or clinical psychologist who is not a member of the Medical Staff, visiting privileges may be granted on a case by case basis to fulfill an important patient care need.

  DEA: Licensure with the Drug Enforcement Administration
  C.V.: Curriculum Vitae, a written account of a provider’s career and qualifications
  ABMS: American Board of Medical Specialties. A provider’s certification with the respective specialty board.
  *Projected Start Date: The date we predict credentialing will be finished, it is calculated by the date the application is entered in MSO, plus 90 days.
  *Requested Start Date: The date the department chair has requested the provider to start clinical activity.
