The official transition from Brown & Toland to the Hill Physician Group is currently underway, the official transition date is January 01, 2010.

All UCSF health care practitioners whose privileges started on or after September 01, 2009 submit the items below.

If you are currently a UCSF physician and not in the Hill Physician directory but want to be:
Hill Physician limits listings in the directory to
full-time clinical faculty, if you meet these requirements and would like to be listed complete the items below.
- Submit a current C.V. (Attach an explanation of gaps greater than six months)
- Complete the Hill Physician Attestation
- Email or Fax (415.476.4150) to the
UCSF Medical Group Credentialing
