Application Process for U.S. Students:

Beginning in 2009-2010, UCSF will only be accepting applications submitted on-line through the Association of American Medical Colleges (AAMC) Visiting Student Application Service (VSAS), available at http://www.aamc.org/programs/vsas/students/start.htm

Application Process for Canadian, Osteopathic Students and International Students (international students -- this is in addition to required forms by the department of radiology)*:

Canadian and osteopathic visiting students may apply to participate in our electives using the following forms (Osteopathic students: use the Canadian application, $150.00 fee, please provide the last 4 digits of your social security #):

• Canadian and Osteopathic Application Form
• Health Form

*NOTE:
Due to the Department of Homeland Security regulations, there will be a $300 processing fee for international students (including Canadian students) covering Services to International Students and Scholars.

If visa documents are delivered by courier, applicants should expect to pay an additional $25 for this service.
