Creating a Formatted Forms Page

We are glad to be able to offer you web-based forms. These forms collect the data for you in excel format. If you are interested in having us develop a web-based form for you, please contact us and we will be glad to discuss it with you.

Once we have determined your needs, we will ask you to mock up your form. Our form-building tool uses standard elements, so you will be required to create your mock-ups based on these standard elements. Your mockup must be complete and signed-off before we begin buildout. No changes may be made to a form once we have begun development.

Building the Mock-up

In the templates folder, at the root of the website, you will find a file entitled _forms.aspx.

Cut and paste these elements into your standard template as you do with any of our cut-and-paste library elements.

Please do not manipulate the formatting of these elements. Our forms creation tool does not allow for any but those on the _forms.aspx template. The only exception to this is that you may add up to 8-10 entries in the checkbox and radiobox fields.
